    Introduction: Spinal meningioma are dural-based intradural extramedullary tumors usually occur in the mid thoracic middle aged women. Extradural spinal meningioma are not well recognised variant.

    Methods: We present a case of spinal meningioma that is exfoliating from the dura and located within the spinal canal exhibiting peculiar radiological features different than that of Spinal meningioma.

    Results: Clinical data, radiological studies, management steps, clinical outcome and long term follow up are presented.

    Conclusions: Spinal dural-based extra dural meningioma is a very rare entity. Recognising The radiological features of this rare entity in preoperative images can help to better plan the surgical resection.
